PLEASE READ, COMPLETE, AND SUMBIT ALL FORMS

PLEASE READ CAREFULLY!!

  • Call if your child is NOT coming.

  • Call if you are planning to be late or if you are picking up early.

  • Inform Staff if someone other than designated persons will pick up your child. *NOTE: Your child will NOT be released to anyone who does NOT have proper identification or authorization. (no exceptions)

  • Provide 2 weeks written or verbal notice BEFORE termination of services.

  • Provide 2 weeks written or verbal notice when child will be out for family vacation.

  • Each family is ONLY allowed ONE holding fee for family vacation, etc

  • Three days is considered a full week

  • If your child does not come, your fee is still due.

Pay tuition fees on Monday for the week, unless other arrangements have been made with Ms. Lisa.

*NOTE: There will be a $5 late fee for EACH day that tuition is NOT paid.

  • Pay a late pickup fee of $3 the first minute and $3 every 15 minutes thereafter (this fee is paid because I’m still working with and responsible for your child.) This is to be paid at the time of pickup.

  • Provide a change of clothes to be left at the summer camp for emergency. (optional)

  • Allow yourself a few extra minutes upon arrival to inform staff of anything that we need to be aware of concerning your child. Examples: not feeling well, a restless night, a fall, etc

  • Parents are responsible for providing lunch each day.

Child Care Provider

As parents, you can expect:

  • A safe and nurturing environment which promotes the individual development of children.

  • Encourage children to become autonomous and enthusiastic learners, providing the, with choices and opportunities to be responsible for themselves, as independently appropriate for their level of development.

  • Use positive guidance techniques in response to inappropriate behavior facilitate self-control through positive modeling, encouragement, re-direction and setting clear limits

In addition, G.L.A.D HEARTS

  • Give written notice of termination of services, of all holidays, vacations, personal development days, and other closings at least 2 weeks prior. (This notice excludes sick days, in which parents will be notified as soon as possible)

  • G.L.A.D Hearts will provide breakfast and PM snack Breakfast 8:30 am - 9:00 am (no breakfast will be served after 9:15)

  • However, we are not responsible for a substitute provider when site is closed.

  • We can NOT administer medication if written permission is NOT signed.
